These terms describe the terms and process for applying for a Confidis
Palliative Membership. By applying, you confirm that you are requesting
membership for your organization.
What membership includes
- Access to the Confidis member library of policies and clinical guidelines
- Use of the Confidis Palliative Assessment Tool
- Access to educational events for palliative administrators and staff
The application process
Confidis uses the approval process to verify your contact information and
organization size. After you apply for membership, Confidis will contact you
to provide pricing information and a licensure agreement that provides the
terms for using our proprietary risk assessment tool for palliative care.
Invoicing and payment of membership fees is handled offline. You are under no
obligation to agree to membership. Your organization will have an opportunity
to review our fees, licensure agreement and terms before becoming a member.
Billing
After your application is approved, Confidis will invoice your organization
for each membership renewal. Payment is handled offline. Submitting this
application does not charge a card and does not complete a checkout.
User accounts and access
Once your organization is approved and has signed a member agreement, the
membership administrator may invite staff logins for people within your
organization. Logins may not be shared with anyone outside the organization.
Response time
Confidis typically reviews applications within two business days and will
contact you by email. Confidis may approve or decline an application at its
discretion.
